Contemporary observers naturally suspected malaria as the cause, given that both individuals exhibited characteristic symptoms such as recurring fevers. Yet the narrative quickly shifted toward conspiracy. Rumors of deliberate poisoning spread rapidly, with Francesco’s younger brother Ferdinando emerging as the prime suspect. As the heir apparent, Ferdinando faced potential displacement from the succession line should Francesco’s illegitimate child, Antonio, be elevated instead. Complicating matters further, Ferdinando had recently visited the grand duke and his wife at their residence shortly before they became ill, fueling suspicions that he administered arsenic to secure his position. The couple’s final days were spent at a Medici villa located in Poggio a Caiano, situated near Florence. This region featured extensive marshlands and rice paddies—environments perfectly suited for mosquito populations capable of transmitting malaria. Despite these favorable conditions for the disease, assassination theories persisted, possibly reinforced by the Medici family’s well-documented history of violent deaths and attempted murders.

Scientific Investigation Through the Ages

Since 2004, the Medici Project has systematically exhumed and examined skeletal remains from forty-nine Medici family tombs. Multiple investigations have since validated malaria as Francesco’s actual cause of death. Nevertheless, research published as late as 2006 employed toxicological methods to conclude that arsenic poisoning had indeed killed both individuals.

The Definitive DNA Evidence

A groundbreaking study spearheaded by Serena Tucci, assistant professor of anthropology at Yale University’s Faculty of Arts and Sciences, and collaboratively executed with the University of Pisa in Tuscany, has finally settled this centuries-old controversy. Utilizing DNA extracted from the bone remains of Francesco and his brother Giovanni, researchers sought to provide conclusive proof.

In recent years, we tried to solve this mystery by performing some specific analysis, in particular paleo-immunological analysis, which attested to the presence of malaria in the remains. But the rumors would not stop, because paleo-immunology is not resolutive, and only ancient DNA could give an answer with a high degree of certainty,

Valentina Giuffra, professor of history of medicine at the University of Pisa and coauthor of the research published in June within the journal iScience, explained the methodology. Paleo-immunology examines antigens—substances that provoke immune responses—or proteins to detect disease markers in ancient specimens. DNA analysis represents a more contemporary technique, offering greater precision by identifying direct genetic signatures associated with specific illnesses. Giuffra and her team discovered genetic evidence of plasmodium, the parasitic protozoa responsible for malaria, within bone samples collected from Francesco’s ribs.

DNA is certain. It solves the problem and the doubts. I think this is a definitive answer.

The genetic examination utilized small bone fragments preserved during the 2004 opening of Medici tombs, before remaining remains were reinterred. Scientists had been unable to conduct comparable analysis at that earlier date because the necessary technology remained insufficiently developed. Remarkably, the investigation identified not merely one but two distinct malaria parasite species—Plasmodium falciparum and Plasmodium malariae—within Francesco’s remains, indicating he likely suffered from a dual infection. Malaria stands among humanity’s most devastating historical pathogens, responsible for 610,000 fatalities in 2024 alone according to World Health Organization statistics. The illness presents with fever, headaches, and chills. Its etymology derives from the medieval Italian expression “mal aria,” translating to “bad air”—a name reflecting the historical belief that the disease spread through breathing contaminated air near swamps or stagnant water bodies. Historical documentation corroborates Giuffra’s conclusions. Court physicians serving the Medici family recorded symptoms aligning with malaria and described therapeutic interventions including bloodletting. This practice involved intentional blood withdrawal, believed at the time to expel illness but which actually exacerbated patients’ conditions.
