“Heatstroke is a medical emergency. It must be treated immediately because it can quickly become fatal. The body normally has the capacity to control and regulate its temperature, but when this system is overwhelmed, the consequences are dire,” Dr. Wen explains.

Common symptoms include confusion, dizziness, fainting, slurred speech, loss of consciousness, and even seizures or irregular heart rhythms. If someone exhibits these signs, they should be taken to the hospital without delay. Bystanders are encouraged to call emergency services and assist in cooling the individual down until help arrives.

How to Cool Down Without Air Conditioning

When heatstroke is suspected, Dr. Wen outlines key steps to help someone until professional medical care is available. Moving the affected person to a shaded, cool area is the first priority. Applying cold, wet cloths or ice packs to the neck, armpits, and groin can accelerate cooling. Soaking clothing in cold water and using fans or evaporative cooling methods, like misting, can also be effective.

“It’s essential to stay with the person until emergency responders arrive,” she adds. “Monitoring their condition and ensuring they remain hydrated can make a significant difference.”

Recognizing Less Severe Heat-Related Illnesses

Before heatstroke develops, individuals may experience heat exhaustion, a milder but still serious condition. Symptoms of heat exhaustion include nausea, fatigue, dizziness, headaches, heavy sweating, and cool, clammy skin. Dr. Wen highlights the importance of moving to a cooler environment and loosening clothing. Taking a cool bath or applying wet cloths to the body can provide relief.

“If symptoms persist or worsen—such as fainting or vomiting—seek immediate medical assistance. It’s also crucial to monitor progress; if there’s no improvement within an hour, further action is needed,” Dr. Wen notes.

Another less severe condition is heat cramps, characterized by painful muscle spasms during physical activity in hot weather. Those affected should stop exercising, rest, and replenish fluids with water or electrolyte solutions. Early intervention can prevent progression to more severe forms of heat illness.

Heat-Related Illnesses Beyond the Immediate

Dr. Wen identifies two primary pathways through which extreme heat impacts health. The first involves direct overheating, while the second occurs when high temperatures exacerbate existing medical conditions. “Prolonged exposure to heat can stress the heart, lungs, and kidneys,” she says. “This interaction is linked to a significant portion of heat-related deaths.”

Researchers estimate that 1 in 4 heat-related fatalities are either directly or indirectly caused by the strain on cardiovascular systems. Chronic conditions like diabetes and certain mental health diagnoses are also vulnerable. For instance, heat can worsen symptoms of conditions such as hypertension or asthma, increasing the risk of complications.

Who Is Most at Risk?

While anyone can suffer from heat-related illnesses, certain groups are more susceptible. Dr. Wen categorizes these individuals into three high-risk populations. The first includes young children and the elderly. Both groups have reduced physiological resilience, making it harder for them to regulate body temperature. Children may struggle to communicate their discomfort, and older adults often experience dehydration more easily.

“The very young and elderly are at higher risk of dehydration, which can compound the effects of heat,” Dr. Wen points out.

The second group comprises individuals with chronic medical conditions. Medications such as diuretics or antihypertensives can impair the body’s ability to adapt to temperature changes. Heat can then place additional stress on already vulnerable organ systems, leading to complications.

The third category includes people facing occupational or socioeconomic challenges. Workers in outdoor environments, such as construction or agriculture, are exposed to heat for extended periods. Meanwhile, individuals without access to cooling spaces—like those experiencing homelessness or living in poorly ventilated homes—may not have the means to escape the heat.

Preventing Heat-Related Emergencies: Practical Steps

Dr. Wen advises that those in vulnerable groups should take proactive measures when heatwaves are forecasted. Staying hydrated is paramount, with a focus on drinking fluids regularly, even before feeling thirsty. She recommends consuming water or electrolyte-rich beverages to maintain bodily balance.

Timing of physical activity is another key factor. Exercising during the hottest parts of the day—typically between 10 a.m. and 4 p.m.—should be avoided. If outdoor activity is unavoidable, taking frequent breaks in shaded areas and wearing loose, breathable clothing can help. Additionally, monitoring neighbors or loved ones, especially in high-risk categories, is vital. Checking on those who may be isolated or unable to access cooling resources can prevent severe outcomes.

Dr. Wen also emphasizes the importance of early recognition of symptoms. “If someone starts showing signs of heat exhaustion, like excessive sweating or fatigue, they should be moved to a cooler environment immediately,” she says. “Prompt action can prevent the situation from escalating.”

Heatwaves need not disrupt summer activities or fitness routines. By implementing strategies such as using fans, staying indoors during peak heat, and ensuring access to shade and water, individuals can mitigate risks. Dr. Wen concludes that awareness and preparation are the cornerstones of preventing heat-related emergencies, ensuring that even in extreme conditions, health and safety remain priorities.
