Researchers from Northwestern University’s Feinberg School of Medicine conducted a study involving 942 primary care patients with an average age of 52. Participants were tasked with simulating a doctor’s visit, where they received a new diagnosis and were asked to summarize the key points afterward. They were also given a fictional prescription bottle to evaluate how well they could interpret the label and instructions. Despite the simplicity of the tasks, many participants faltered, suggesting a gap in health literacy that affects even those familiar with the healthcare system.

“Our study was able to really provide some validation to people,” said Abigail Vogeley, a research fellow and doctoral student at Northwestern. “They’re not alone in being confused by the healthcare system and by medications. It’s something that us, as the healthcare system, need to improve and help people with rather than just saying, ‘OK, good luck.’”

The results underscored how subtle variations in labeling and language can lead to misinterpretation. For instance, a single phrase like “take twice daily” might be read as a single dose split into two parts, or as two separate doses taken at different times. Such ambiguity raises concerns about whether patients are truly adhering to the intended treatment plan.

The Impact of Misunderstood Instructions

Health misinformation is on the rise, and the study’s findings align with this trend. Dr. Michael Wolf, professor of medicine and director of Northwestern’s Center for Applied Health Research on Aging, emphasized that the complexity of medical language often hinders comprehension. “How do you sustain that behavior if you’re literally having to take medications up to seven times a day, which, quite honestly, is what we see people doing,” Wolf noted.

The study also found that unclear instructions can result in patients overcomplicating their medication routines. For example, someone might take a diuretic at night instead of morning, leading to frequent bathroom trips during sleep. Stefanie Ferreri, professor of pharmacy practice at the University of North Carolina at Chapel Hill, highlighted that such errors can be avoidable if instructions are more explicit.

“People wanted to get their prescription information right and do the best they could for their health,” said Dr. Jan Carney, president of the American College of Physicians and professor at the University of Vermont. “But fostering health literacy can be difficult when doctors’ visits are short and the information may not be written in a straightforward way.”

Communication gaps between healthcare providers and patients are a significant factor in these misunderstandings. Carney pointed out that many patients hesitate to ask questions during appointments, fearing they may appear uninformed. This reluctance can compound the problem, as unclear instructions are often left unchallenged.

Addressing the Root Causes

Abigail Vogeley added that the issue stems from a lack of standardization in how medical information is presented. “Some of the warnings are maybe written differently or in different places that patients don’t think to look,” she explained. “It’s a uniformity problem.” This variability in labeling practices can confuse patients, especially those who are not accustomed to reading detailed medical directions.

The study also revealed that patients may misinterpret the purpose of their medications. For instance, a drug meant for morning use could be taken with dinner, altering its effectiveness or increasing side effects. Such errors can have serious consequences, from reduced treatment efficacy to adverse health outcomes. Wolf stressed that medication generally works best when taken as instructed, but this requires clear, accessible communication from healthcare professionals.

Researchers argue that improving health literacy is not just about individual effort but a systemic responsibility. “The results of the study are not an indictment on the individual, but rather a call to action for the health system to enhance communication and collaboration with patients,” Wolf stated. This includes simplifying language, using consistent formatting, and encouraging patients to ask questions without hesitation.

Practical Solutions for Better Understanding

Experts suggest that small changes could make a big difference. One key recommendation is to ask patients a single, focused question about their medication after receiving instructions. This approach ensures that the most critical information is reinforced and understood. For example, instead of asking, “Do you understand how to take your medication?” which may be vague, healthcare providers could ask, “When should you take this medication with food, and how often?”

Additionally, the study highlights the importance of patient-centered education. Dr. Carney noted that doctors often prioritize speed during visits, leaving patients with fragmented information. By dedicating time to explain instructions in simple terms and check for understanding, healthcare providers can reduce errors. This might involve using visual aids, analogies, or written summaries to clarify complex details.

While the study focused on medication management, its implications extend beyond prescriptions. Health literacy affects a wide range of tasks, from interpreting lab results to following lifestyle recommendations. Vogeley emphasized that improving this skill can empower patients to take control of their health, leading to better outcomes and fewer preventable mistakes.

The findings also resonate with the growing trend of health misinformation. As patients increasingly rely on online resources and word-of-mouth advice, the need for accurate, clear communication becomes more urgent.
